OKLAHOMA PARDON AND PAROLE BOARD PARDON APPLICATION CHECKLIST
1. Complete, Sign, and Notarize the Certification and Personal Oath
2. Obtain Certified Copies of Judgement and Sentence on Each Convictions
3. Obtain Certified Statement from Court Clerk
4. Obtain Current Credit Report
5. Provide Current Proof or Verification of Employment or Income
6. Provide Current Proof or Verification of Residence
7. State Issued ID
8. Provide copies of military separation documents (if applicable)
9. Provide copies of military court-martial promulgating order (if applicable)
10. Provide copies of documents relating to any denial, suspension or revocation of Occupational License (if applicable)
11. Character Affidavits (Optional)
12. Complete the Pardon Application
If confirmation is needed that an application was received by the Pardon and Parole Board, the documents should be sent via certified mail as this will provide confirmation.
APPLICANTS SHOULD KEEP A COPY OF THE COMPLETE APPLICATION PACKET AND A COPY OF ALL ATTACHMENTS!
